  1. Confirm your slots by emailing the UP SLIS College Secretary’s Office at collegesecretary [AT] slis [DOT] upd [DOT] edu [DOT] ph. Use subject:  BLIS Accepted Applicant Confirmation / 2S 2016-2017 / SURNAME Accepted applicants must confirm their slots on or before Monday, 2 January 2017. Failure to do so will forfeit your chance to be admitted to the BLIS Program.
  2. Process your admission credentials on T-W, 3 & 4 January 2017. Kindly obtain the following from your home unit:
  • Permit to transfer - original & photocopy
  • College Clearance - original & photocopy
  • Student Directory – get forms from Ate Oda on 3 Jan 2017
  • TCG – updated with 1st sem 2016-2017 grades, original & photocopy
  • UHS Medical Certificate for T1 only – present medical exam results if you have had one in the past 6 months and have them validated at the UHS
  1. Those with student numbers from 2010 and below, kindly write a letter of appeal for MRR extension, addressed to Dean Kathleen Lourdes B. Obille, for submission with the rest of the requirements.
