   Accepted applicants must confirm their slots by 11 JUNE 2018. Failure to do so will forfeit your chance to be admitted to the BLIS Program.
   
   
2. Process your admission credentials ASAP / by 22 June 2018.
   
   Kindly obtain the following from your home unit upon claiming your NOTICE of ADMISSION from Ate Oda from 13 June 2018 onwards:
   
   
   1. Permit to transfer - original & photocopy
   2. College Clearance - original & photocopy
   3. Student Directory – form attached here, for printing on pink A4 paper
   4. TCG – updated with 2nd sem 1 7 -18 grades, original & photocopy
   5. T1 only: UHS Medical Certificate – take routine Med Exam or, present medical exam results if you have had one in the past 6 months and have them validated at the UHS
      
      
3. Those with student numbers from 2012 and earlier, kindly write a letter of appeal for MRR extension, addressed to Dean Kathleen Lourdes B. Obille, for submission with the rest of the requirements.
