Use subject: BLIS Accepted Applicant Confirmation / 2S 2017-2018 / SURNAME
Accepted applicants must confirm their slots on or before 3 January 2018. Failure to do so will forfeit your chance to be admitted to the BLIS Program.
- Process your admission credentials on ASAP.
Kindly obtain the following from your home unit upon claiming your NOTICE of ADMISSION from Ate Oda on 3-4 January 2018:
- Permit to transfer - original & photocopy
- College Clearance - original & photocopy
- Student Directory – form attached here
- TCG – updated with 2nd sem 17-18 grades, original & photocopy
- T1 only: UHS Medical Certificate – present medical exam results if you have had one in the past 6 months and have them validated at the UHS
- Those with student numbers from 2011 and earlier, kindly write a letter of appeal for MRR extension, addressed to Dean Kathleen Lourdes B. Obille, for submission with the rest of the requirements.
